Ongoing evaluation: outputs
The outputs are an updated profile and an updated evaluation document with the monitoring list, the procedure, the recurrence rationale and the drift plans.
What clause 9.3.3 asks
Two things come out: a new profile version and an updated evaluation report. The report holds four items: what is watched, how each item is checked, why the checks repeat when they do, and every drift found with its treatment plan, if there is one. These four make the watching auditable.
Why it matters
The recurrence rationale is the item most often missing. A monitoring plan that says monthly, with no reason, is a habit rather than a decision. The clause allows recurrence by time or by other indicators, such as transaction volume, throughput, automatic alarms or unusual conditions. The plan for each drift can be none, but the decision that none is needed has to be recorded.
What good looks like
Produce the four items as one document. For each monitored item, record the procedure, the recurrence and the indicator that would trigger an earlier review. When a drift is found, record the evidence, the plan and the owner. When no plan is needed, record that decision and its reason. Keep the document versioned with the profile.
Evidence an assessor asks for
The assessor will ask why the review interval is what it is. Monthly because that is the calendar is not a rationale.
- Monitoring list Each item, its procedure and its owner
- Recurrence rationale The interval or indicator and the reason for it
- Drift record Each drift found, its evidence, the plan and the owner
- No-plan decision Each drift with no plan, and the reason
Where it commonly fails
- The recurrence has no rationale
- A drift is found with no plan and no recorded decision
- The document is not linked to the profile
What to do before publication
Write the recurrence rationale with the operations lead, using the system's real change rate.
